Forms

These are some forms I have created to make my life easier!  I hope you get some use out of them as well!

Mini Economy!

Although my formal mini-economy class isn’t until next week, I did go ahead and make a few of the forms I knew I wanted to use this coming school year.  I plan on each of my students having a job that they have to do at least once a week.  They will need to fill out a job application with the help of either their parents, or a classroom parent helper, or my assistant.  After that, I will interview them and see if they are right for the job.  I have also attached a list of jobs in the “application” document that I will be using.  I came up with 30+ jobs, so if some jobs are seasonal (zookeeper, plant watering person, etc.) then there would be extras to fill in those gaps.  I think during 2nd semester, we might re-interview for our jobs if anyone wants a new job.  They will be paid for their jobs and keep their money (“Bulldog Bucks” since our mascot is the Bulldog) either in the bank or in the pouch in their binders.  On Friday’s we have a time for the banking to occur, as well as payday and store.  I am fortunate enough to have access to Teachers Treasures and can “shop” once a month for free materials for my classroom.  Many of the items that I am able to get from TT I can use as items in my store.

This is a listing of themes my team made several years ago at one of my other schools I am no longer at. It is a monthly listing and included a specific letter(s) of focus for the month.  I now include letter instruction on a daily basis and would encourage others to drop the “letter of the week” and look into integrating their letter/sound teaching by using and being true to Building Blocks and that frame work–it’s so much easier and takes much less time and copier preparation and the kids have so much fun!

Story Sacks

These are the parent information sheets I created for my Story Sacks.  Each child gets a sack once a week and keeps it until Thursday.  The sheets tell the parents what to do with the materials inside the bag.  I use these sacks for my advanced students when they have completed a DRA level 8 or above.

Small Group Language Instruction

I created this sheet during my student teaching (many years ago!) to help my assistant know what to do with the children who were still struggling with letter recognition.  I put the items in a large laundry basket and she took a group of 4-5 students and worked on 1-2 activities with them each day for 20 minutes.

This is a behavior form I made to track the daily behaviors of some of my students.  This gets sent home at then end of the day and signed by the parent and then is returned.  When they are returned, I file them and use them for an artifact at our staffing if needed.

This form is used to sign families up for Kindergarten conferences twice a year.  I send it home and they pick the three best times that work for them and return in.  I assign spaces in the order that I receive the forms back-return it quickly=time you want! :)

I couldn’t find a lesson plan book to suit my needs, so I made this grid up instead.

These are the directions and forms my parent volunteers use when they come into help.  I have the games in plastic shoe boxes and the directions all typed up and placed inside with them.  I send groups of 3-4 children at a time with a parent. When they are done, they complete this little 1/2 sheet.  I use it to track who works well with whom, their levels of learning, and how they behaved.  This year, I will pick up all the sheets monthly and draw one name out and send that parent a little thank you gift (starbucks card, calendar, planner, etc.)

Crate where 1/2 sheets are stored    Parent Volunteer Games                    Parent Volunteer Board

My parents would sign up a month or so in advance on a calendar I had a the Parent Volunteer Center.  On the day they were to be there, they would look at the board and find the day of the week that it was.  Under the day of the week, I stapled a Ziploc bag.  In the Ziploc bag, there was a card listing the activity for the day.  I also stapled a paper cup to the board and labeled large wood sticks with each child’s name.  Every morning I had a helper, I would choose 3-4 children and put their sticks in the cup.  When the parents came in they would call the childrens’ names and take the days activity and go–It was easy and provided minimal distraction.  I would have them work for about 20 minutes with a small group and when they returned, I would have them work with another group that was ready to go.

I’ve had MANY parents ask me over the years to send home something to do with their children over Spring Break so that they stay in “school mode”.  I send home this calendar of things to do with them–I kept it simple and easy!

  1. Thanks you so much for the great forms section. I have chosen a few that I am going to try this year. I especially like the parent volunteetr section.

  2. I absolutely LOVE the small group language instruction sheet for assistants. I will use this sheet this year for my new assistant!! I’ve told my previous assistant these same ideas but never had them on a sheet day by day! I look forward to other items on your website! Thanks so much! Lisa

  3. Hi Lisa,
I hope you get lots of use out of that small group phonics sheet–it’s one of my very favorite things I’ve created! :)

  4. Thanks for all of your wonderful ideas. I’m kindergarten dual language teacher here in Albuquerque and most of your ideas I will translated for my 90/10 class. Please feel free if you need any of your forms in Spanish.
